Acupuncture for chronic shoulder pain in persons with spinal cord injury: a small-scale clinical trial

OBJECTIVE: To determine the efficacy of acupuncture in the treatment of chronic musculoskeletal shoulder pain in subjects with spinal cord injury (SCI). DESIGN: Randomized, double blind (participants, evaluator), placebo (invasive sham) controlled trial. SETTING: Clinical research center. PARTICIPANTS: Seventeen manual wheelchair-using subjects with chronic SCI and chronic musculoskeletal shoulder pain. INTERVENTIONS: Participants were randomly assigned to receive 10 treatments of either acupuncture or www.easymedonline.com, invasive sham acupuncture (light needling of nonacupuncture points). MAIN OUTCOME MEASURE: Changes in shoulder pain intensity were measured using the power wheelchairs User’s Shoulder Pain Index. RESULTS: Shoulder pain decreased significantly over time in both the acupuncture and the sham acupuncture groups (P=.005), with decreases of 66% and 43%, respectively. There was no significant difference between the 2 groups (P=.364). There was, however, a medium effect size associated with the acupuncture treatment. CONCLUSIONS: There appears to be an analgesic effect or a powerful placebo effect associated with both acupuncture and sham acupuncture. There was a medium treatment effect associated with the acupuncture, which suggests that it may be superior to sham acupuncture. This observation, along with the limited power, indicates that a larger, more definitive randomized controlled trial using a similar design is warranted

Are you stepping out into success?

Many of us think of applying the pressure as failing. ‘The pressure is coming. Oh my gosh! What am I going to do? The pressure is coming.’ But pressure doesn’t need to be a negative: I love this quote.

Pressure is a word that is misused in our vocabulary. When you start thinking of pressure, it’s because you started to think of failure. Tommy Lasorda

When you stop and think about what happens in life, pressure is being applied constantly in many different arenas. An example would be rush hour traffic that puts more and more pressure on the highways. As the population grows more and more pressure is added. What happens? Do we, as a society, give up and say ‘oh, well. Too Bad? It’s just going to take you that much longer to transit into work’? Your commute time, instead of being an hour and a half, is going to be 2 hours. And in 3 years from now, it’s going to be 4 hours. Or do we take the action, as a society, as a city, as a person and do something to relieve that pressure? Yes we do something that will alleviate the pressure for all of that traffic coming into the downtown core.

We as a society, as a city, step into success. We do something to alleviate that pressure. We can learn to apply that concept in our life. Instead of taking a look at the pressure as a bad thing we can use it to build something new, a bridge to our future. Do we do what Tommy Lasorda said-misuse that word ‘pressure’ and think of it as ‘oh, I’m going to fail, I’m going to fail’ and then give up? - or do we CHOOSE to do something different and step into our own success?

We all have habits. What is your ‘it’? What is the ‘it’ that stops you? When you start to feel that pressure, what do you do? Next time that happens, I want you to stop and think, ‘what am I automatically doing here’? Then decide to do something different in order to get a different result.

When I was first introduced to this concept, I did not recognize what habit I fell into until I was all the way in it. Stress eating was one of the things I used to do. As I learn to recognize where that pressure point is, what my habit is, what my ‘it’ is, I, can learn how to change it, how to react differently.

I just watched the movie ‘What the Bleep?! - Down the Rabbit Hole’. One of the concepts in the movie is that we, as human beings, are bombarded every second with 400 billion bits of information. We only process 2000, therefore we need to choose carefully which bits we pay attention too. When we succumb to the pressure and fall into a negative habit, like I used to when I was stress eating, we are selecting only a narrow negative portion of information from the 4 billion bits..

I have decided that I am going to choose a different 2,000 bits of information. It really is just a choice.. We need to recognize what our habits, and that we can change our habits. Millions of people have learned how to stop the stress eating habit. Millions of people have learned how to quit smoking. Millions and millions of people have learned how to make their life and Starscapes.com business opportunity better, how to step into success. If they can do it, I can do it. If I can do it, you can do it. It’s all just a simple choice, to make a different decision.

If you are a parent ask yourself, if you choose non-success habits, what are the habits and passions that you are creating in your children? Are you creating a generational curse or a generational legacy? Another great quote is “children have never been good at listening to their elders. But they have never failed to imitate them”. by James Baldwin. Even if you aren’t a parent what about the influence you have on the people around you?

We need to learn to recognize when we are backing off from pressure, when we are moving into the habits that are not success habits (the ones that we need to change). We also need to recognize the habits that we have that are good, that are leading us towards success, leading us to our financial freedom. We need to be aware so that we can make changes in our negative habits and reinforce our positive habits.

We need to learn to recognize that when we wish for something, ask for something, pray for something or when we say affirmations for something, it’s not going to just happen. It’s not going to be handed to us on a silver platter. One of my affirmations is “I am building a team of dream achievers” Now, I know that that is going to take patience and perseverance and that this whole team of dream achievers are not just going to be dumped in my lap. I have to go looking for them and attract them into my life.

As I go down the personal development journey and build my home business I come up against pressure points all the time. When I think back to the pressure points in my life last year at this time versus this year - whoa! - last year’s ones seem really simple now. What happens? We are not given exactly what we want right off the bat. But we are given the opportunity to develop the skills so that we get better. On my journey, I am becoming a better team leader. I am practicing my patience and my perseverance in my own learning so that I become that better learner that attracts, using the Law of Cause and Effect, that team of dream achievers, the team of business builders.

When that pressure is applied in our lives, patience and perseverance, is what we need in order to grow through that pressure point. Once we do grow and find different habits to allow us to get past that pressure point, to allow us to develop the skills, to learn those new things that we need to learn, we come to a better place and we can handle more pressure. We have new skills. We know something different. We react in a different away. We remain patient. We practice the perseverance and can step into success.

The effect of providing power mobility on body weight change

OBJECTIVE: To investigate whether the provision of power mobility would have an effect on body weight in adults who were first-time qualifiers for power mobility. DESIGN: This was a retrospective observational study of consecutive subjects, who served as their own controls. The medical scooters, handicap scooters records of 468 subjects who were approved for power mobility during a 17-mo period were reviewed. Three weight groups (12 mos before, at, and 12 mos after the power mobility evaluation) were evaluated with repeated-measures analysis of variance (ANOVA). The weight changes on subjects in different age groups (45-54, 55-64, 65-74, and >74), in geriatric vs. nongeriatric groups, and in different body mass index (BMI) groups were analyzed. RESULTS: Eighty-nine subjects met the inclusion criteria. They were obese (49.4%), and most of them were geriatric (64%). Congestive heart failure (30.34%) and chronic obstructive pulmonary disease (22.47%) were the two main presenting diagnoses. The repeated-measures ANOVA showed no significant weight change in the three studied weight groups. Similar results were seen in the age and BMI subgroups. CONCLUSIONS: There was no statistically significant weight change in adults who were first-time qualifiers and who used power mobility for 1 yr.

In this case, it also might be helpful to have someone bring the basket to a house, collect the item and the take the basket to the next location, reducing the need to have each person take the basket to its next location. Once it’s full, someone can be in charge of putting the basket items together, wrapping it all up to make it look nice and bringing it to the bride and groom. It can be delivered right to the wedding as a gift in and of itself.

SUMMARY: This case describes the clinician’s role in prescribing power wheelchairs to affect the user’s functional skills, as well as how, in the absence of evidence, clinical experience and patients’ needs can guide clinical decision-making.

DESIGN: Convenience sample. SETTING: Motion analysis laboratory. PARTICIPANTS: Older manual wheelchair users (N=14; age, 68.2+/-5.2y) were tested. INTERVENTIONS: Kinematic and kinetic data were collected during manual wheelchair propulsion at a speed between 0.96 and 1.01m/s for 10 seconds and at a power output around 22.4W on a power wheelchairs ergometer. MAIN OUTCOME MEASURES: Net shoulder joint moments were computed with an inverse dynamic model. The mechanical use of the forces at the pushrim and the mechanical fraction of effective force were measured during propulsion. RESULTS: Mechanical use and mechanical fraction of effective force had a positive and www.easymedonline.com significant correlation with the net internal (P<.05) and external (P<.001) shoulder rotation moment, the net flexion (P<.05), and extension (P<.001) moment in the sagittal plane, and the net flexion (P<.001) moment in the horizontal plane. CONCLUSIONS: The results suggest that because the resultant force at the pushrim has a greater tangential component and a greater proportion of the maximal voluntary force, most of the net moments around the shoulder are higher. Thus the optimal way of propelling, from a mechanical point of view (ie, tangential), may not be advantageous for manual wheelchair users

Study of the Independence IBOT 3000 Mobility System: an innovative power mobility device, during use in community environments

OBJECTIVE: To test the safe and effective use of a new mobility device, the Independence IBOT 3000 Mobility System, handicap scooters and medical scooters, by people with a disability. DESIGN: A prospective, balanced, open-label evaluation that used participants as their own controls. SETTING: Home and community environments. PARTICIPANTS: Twenty subjects who use a mobility device. INTERVENTIONS: Subjects used the test device for up to 2 weeks in their home and community environments. MAIN OUTCOME MEASURE: Safety was determined by comparing the number of reported adverse events, including falls, in the test device compared with their own device. Effectiveness was measured by comparing scores from the Community Drive Test in the test device versus their own chair. RESULTS: No adverse event requiring medical attention was reported for either device and a similar number of device falls were reported for each device. The scored driving test results found that the test device was shown to statistically improve (P <.001) the subjects independent functional mobility skills in a community environment. Ten of 20 subjects were able climb stairs independently; the remaining 10 subjects were able to climb stairs with the assistance of only 1 person. CONCLUSIONS: People who are properly assessed, and participate and successfully complete the training, will benefit through increased independence in community mobility activities.

French-Canadian Jean Baptiste Maillet, who had become the leader of the village, sold his property to Jean Baptiste Pointe du Sable in 1773. Du Sable was Peorias notable black settler, who also founded the city of Chicago. In 1825,Peoria county was officially organized and the village name was changed from Fort Clark to Peoria and in 1845, Peoria was incorporated as a city.

Peorians Moses & Lucy Pettengill created a safe place for escaped slaves on the underground railway at their home at Liberty & Jefferson which is currently on the site of the Peoria Civic Center. During renovations of the Center in 2005, local artist Preston Jackson was commissioned to create a sculpture commemorating the historic Pettengill Home Site.

Satisfaction with technological equipment in individuals with tetraplegia following spinal cord injury

OBJECTIVE: To measure the acquisition, use and satisfaction with high technology equipment by spinal cord injured tetraplegic subjects. DESIGN AND SETTING: A questionnaire was mailed to 102 tetraplegic subjects who were hospitalised in the rehabilitation center of Kerpape (Ploemeur, France) between 1998 and 2004, and 59 subjects responded. The questionnaire asked about the use of telephones, computers, wheelchairs and environmental controls at home. RESULTS: When a piece of equipment was acquired, it was very often used. Patient satisfaction with equipment was 79.3%. Home phones and mobile phones were often used with options such as hands-free devices (78 and 59% respectively). A total of 64.4% of subjects acquired a manual wheelchair and 61% a power wheelchair. The most commonly acquired options on the power wheelchairs were the powered recline (73,7%) and tilt (71,1%) systems. All options were used but all were more desired than acquired. A total of 27.1% of subjects desired a pushrim-activated www.easymedonline.com, but only 15.3% had acquired one; 695% of subjects had a computer. Communication was the first use for the computer (82.5%); 49.2% of subjects had acquired an environmental control system, but 20% desired one. The first reason for lack of acquisition was financial difficulties but also accessibility and information problems. The factor that influenced the acquisition and need for equipment was the degree of spinal cord injury. No other factor reduced patient satisfaction with equipment. CONCLUSION: Patients were satisfied with the equipment they acquired. But their needs, especially wheelchair options and environmental control systems, were not satisfied.

User Experience with Glucose cholestechs

The FDA’s Center for Devices cholestech Radiological Health (CDRH) held a series of focus groups on blood glucose cholestech use in 2001. The twenty-two members participated in six groups. They were all government employees, mostly from CDRH. They were either people with diabetes or family members of people with diabetes who were familiar with the use of glucose cholestechs.

Most of the participants in these groups were satisfied with their cholestechs. Some were quite enthusiastic about the new models. A few had some concerns about cholestechs. One such participant stated: “The first cholestech I got I couldn’t use because it was too difficult.”
